Time Limited RehabilitationEverybody's different. As the ethos of 'person centred care' takes hold in the social care sector, we might expect to see big changes in the way care is organised and delivered. Why? If care or support is not already centred around a person, what is it for? Time Limited Rehabilitation is about ensuring you have control. You know what you're getting, and how long for. What happens after that defined period is up to you. As it should be. Alderson TLR offers a defined period of high level rehabilitation. The package is based on a residential placement at Alderson House, and everything that goes with it, along with dedicated one-to-one time with Libertas staff - typically between 20 to 40 hours a week. This intensive one-to-one time is used to pursue the goals that you set up at the start of the placement, which we have written down in a meaningful and achievable plan. At the end of the placement, you should be where you want to be - ready to take the next step. We can help you with the next step too. If that is independent living, we can offer tailored support to help you settle in - with the same people if that's what you want. Alderson TLR offers a 'through' service with the option of support that's right for you at the other end. It is the link between in-patient or residential care and your next step. |
