Time Limited Rehabilitation

Everybody's different.  As the ethos of 'person centred care' takes hold in the social care sector, we might expect to see big changes in the way care is organised and delivered.  Why?  If care or support is not already centred around a person, what is it for?

Time Limited Rehabilitation is about ensuring you have control.  You know what you're getting, and how long for.  What happens after that defined period is up to you.  As it should be.

Alderson TLR offers a defined period of high level rehabilitation.  The package is based on a residential placement at Alderson House, and everything that goes with it, along with dedicated one-to-one time with Libertas staff - typically between 20 to 40 hours a week.  This intensive one-to-one time is used to pursue the goals that you set up at the start of the placement, which we have written down in a meaningful and achievable plan.  At the end of the placement, you should be where you want to be - ready to take the next step.

We can help you with the next step too.  If that is independent living, we can offer tailored support to help you settle in - with the same people if that's what you want.

Alderson TLR offers a 'through' service with the option of support that's right for you at the other end.  It is the link between in-patient or residential care and your next step.

TLR Illustrations

The concept and working of a Time Limited Rehabilitation placement is perhaps best described using examples.  Alderson TLR is designed to meet a need, rather than have a need shoe-horned into a service, so don't take the illustrations as indicative or limiting;  how you use TLR is up to you.  The narratives are of course, entirely fictional.

John has been receiving in-patient care for a number of months.  He has benefitted greatly from the stabilised medication regime, occupational therapy input and first class nursing care.  John now feels that he is ready to move on, but is a little concerned about coping on his own.  He definitely does not want to move in to a residential home.

Judith has been an in-patient for a long time.  She has tried several residential homes, but these have invariably broken down after a few months.  Judith knows the ward staff, is comfortable with her lifestyle, and remembers all too well the fuss and emotion associated with a placement breakdown.  Judith is not greatly taken with the idea of moving again, but with the help and support of her team and named nurse, she is willing to give it a go.

Caveat

Alderson TLR is designed to help meet the needs of someone who wants to take the next step, whatever that step may be.

We must be mindful however, that everyone is different.  We will do everything we can to help someone succeed, but we cannot of course, guarantee an outcome.  Ambition and desire are the finest motivators to success, but we have to temper ambition with some sense of realism.
